Initial Comments:
An offsite follow-up survey conducted on December 8, 2021, found that Clearfield Dialysis, had corrected the following deficiency cited under 42 CFR, Part 494.62, Subpart B, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services-Emergency Preparedness. The standard level deficiency was cited as a result of a Medicare recertification survey that was completed on October 28, 2021.

An offsite follow-up survey conducted on December 8, 2021, found that Clearfield Dialysis had corrected the following condition and standard level deficiencies cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiencies were cited as a result of a Medicare recertification survey that was completed on October 28, 2021.
As a result of the survey, Clearfield Dialysis was identified to have the following standard level deficiencies that were determined to be in substantial compliance.
